The former Geelong Falcons mentor, who played for the Reid Oval club before he was drafted to Sydney, will take on a development coach role under newly-appointed mentor David Teague.
O'Keefe has a distinguished eight-year coaching career, working with junior talent at Geelong Falcons and most recently as an assistant coach with the Geelong Cats in the VFL.
Carlton's head of football Brad Lloyd said O'Keefe was a welcome addition to the Blues' ranks.

"We have been able to really strengthen the off-field team at our football club for next year and we feel the new faces will suit our already strong coaching group," Lloyd said.
"This coaching group is really diverse with experience across many different areas. We feel this perfectly complements our playing group and provides great support for David Teague.
"All our coaches will be pushing to get the best out of our players by maintaining high standards as we strive for consistent on-field performance in 2020."
